When is the deadline to renew or enroll in a Triple-S Salud plan?

The renewal or enrollment period ends on May 16, 2022. The coverage will be effective from June 1, 2022 until Mary 31, 2023.

What should I do to do to enroll in Triple-S Salud health plan?

    If you’re currently enrolled with Triple-S Salud, you will receive a plan change form by mail. To make changes to the new products we offer and/or add any additional coverage, you can do so by completing the change until May 16, 2022.

    The new policy year begins from June 1, 2022 to May 31, 2023.

What should I do to do to enroll in Triple-S Salud health plan?

    For your convenience, a representative of Triple-S Salud will be available to help you complete the Enrollment Form* in all its parts. You can call (787) 728-1039 for this assistance. You will have to hand in a signed copy of the enrollment form to the Human Resources Department of your Agency or Municipality. If you are an employee of the Department of Education, you must deliver the Enrollment Form to the different school districts you work in for processing. Remember, you have until May 16, 2022.

To expedite your plan subscription, it is important you have the following documents available:

  • Complete the cancellation form SC-1330.
  • If you have couple contract (both employed by a government agency), you must include the SC-1335 Form
  • Payment stub that certifies the agency for which you work.
  • If you want to include dependents in this contract, you must provide:
    • Birth certificate
    • Marriage certificate
  • If you request a supplementary coverage for you or a dependent, submit a copy of the Medicare card with Parts A and B.

If you want to change to a different health plan offered by Triple-S Salud, visit our Service Centers.

  • Service Centers: Plaza Las Américas Monday to Saturday from 9:00 AM to 6:00 PM and Plaza Carolina Monday to Saturday from 9:00 AM to 6:00 PM Sundays from 11:00 AM to 5:00 PM.
  • Island wide Service Centers:: Caguas, Ponce, Mayagüez and Arecibo Monday to Friday from 8:00 AM to 5:00 PM.
  • San Juan Main Office: Monday to Friday from 8:00 AM to 5:00 PM.
